1 Click "Create" button on Poll Page.

2 Choose Survey.

3 Input the Survey title.

4 Choose the type of poll.

5 Input the whole Question and options.

6 Add another questions (if any) and repeat step 3 and 4.

7 Assign the survey to a group (if any).

8 Press "Create".
